Catasauqua Area School District requires parents/guardians to update their emergency contact and health information forms in the PowerSchool parent portal. Health/medical information and emergency contacts must be provided for each student.
These online forms provide pertinent information in case of an emergency. Completion of the health information form authorizes your child to receive emergency first aid in the event of sudden illness or accident. It also provides notifications if the information can be shared as needed.

Parents/guardians of Lehigh Career and Technical Institute students who need to carry or use medication during the school day should complete a medication form and return it to LCTI. If medication is necessary for a chronic condition, a new form should be submitted each new school year.
With the exception of diabetes supplies, EpiPens and inhalers, student medications must be brought to the school by a parent/guardian, along with written authorization from a health care provider.

Parents/guardians of LCTI students who are susceptible to anaphylaxis and carry EpiPens should complete an anaphylaxis emergency plan and submit it to LCTI.
