Matthew W. Polkowski, 65, of Slatington, died Oct. 30, 2021, at St. Luke’s Hospital, Allentown.

Born in Jersey City, N.J., he was a son of the late George R. and Mary M. (Lord) Polkowski.

He graduated from Whitehall High School and attended Lehigh County Community College.

He was a unit clerk at St. Luke’s Hospital, Lehighton.

He is survived by a son, Sean M. Polkowski of Slatington; two brothers, Michael and Mark, both of Whitehall; and a former wife, Carol L. Polkowski of Walnutport.
