Matthew W. “Matt” Polkowski, 65, of Slatington, died Oct. 30, 2021, in St. Luke’s Hospital, Allentown.

Born Aug. 13, 1956, in Jersey City, N.J., he was a son of the late George R. and Mary M. (Lord) Polkowski.

A graduate of Whitehall High School, he later attended Lehigh County Community College.

He was a unit clerk at St. Luke’s Hospital, Lehighton.

He enjoyed hiking, walking, playing tennis and vacationing at Cape May, N.J.

His survivors include a son Sean M. Polkowski of Slatington; brothers Michael and Mark, both of Whitehall; and former wife Carol L. Polkowski of Walnutport.
