Fellowship Manor, Whitehall, receives top certification
Fellowship Manor, Whitehall, has been awarded Post-Acute Certification from the Joint Commission.
Fellowship Manor is the only nursing care center in the Lehigh Valley and one of two in Pennsylvania to receive Post-Acute Care Certification from the Joint Commission.
The standards of Post-Acute Care Certification highlight the importance of effective care management with the goal of providing quality and safety of care, treatment and service.
Post-Acute care requirements include:
The use and monitoring of clinical practice guidelines, established or specially developed protocols to guide the provision of post-acute care services;
Documented training that helps staff identify early warning signs of a patient’s change in condition and how to respond to the decline;
Dedicated, qualified and competent staff members responsible for the provision of post-acute care services;
Established communication criteria and processes to facilitate effective transitions in a patient or resident’s care, and
Data collection and performance monitoring relevant to reducing patient re-hospitalization.
