The Pennsylvania Academy of Family Physicians is accepting nominations to choose its 2015 family physician of the year – the finest family physician in the state.

Organizers are looking for an extraordinary doctor who goes above and beyond to ensure the very best patient-centered care. For example, perhaps the doctor is always available by phone or email to address concerns; perhaps the doctor makes house calls, keeps extended hours or treats multiple generations of a family. That is the kind of physician who would make a perfect candidate for this award.

The nominee must be a practicing member of the Pennsylvania Academy of Family Physicians.

The deadline for nominations is Jan. 19, 2015.
